At ProRehab, we believe that long-lasting pain relief is possible without expensive surgeries and unnecessary medications.
That’s why every single one of our physical therapists have Masters or Doctoral level degrees, and most hold more certifications like Orthopedic Clinical Specialists (OCS), Sports Certified Specialists (SCS), and Certified Hand Therapist (CHT).
What do all those letters at the end of their names mean? They mean we’re obsessed with providing our patients with a level of care you can’t find anywhere else.

Madison Eubanks PT, DPT

Physical TherapistDr. Madison Eubanks received her Bachelor of Science Degree in Kinesiology from the University of Kentucky. She then went on to earn her Doctorate in Physical Therapy at the University of Kentucky in 2019.
Madison has a background in outpatient orthopedics and joined ProRehab to pursue her passion in pelvic health. Madison is committed to specialized treatment of pelvic health for all individuals. Her professional goal is to achieve a pelvic health specialist certification.
She is originally from Lexington, KY. Go BBN! Madison and her husband Jon now live in Shelby County outside of Louisville with their dog Otto. She loves watching the NFL and cheering on the Kansas City Chiefs. Outside of her career, she enjoys hikes with her family, spending time at Kentucky Lake & traveling the world.
Amy Seger PT, DPT

Physical TherapistAmy Seger is a Ferdinand native and graduated from Forest Park High School in 2014. She now lives in Santa Claus with her husband. She obtained her doctor of physical therapy in 2023 from Bellarmine University, and prior to her graduate career, she graduated from IUPUI with a Bachelors of Science degree in December 2019.
Amy is interested in neuro-rehabilitation and vestibular rehabilitation.
When not at work, Amy enjoys spending time with family and friends, reading, fishing or being outside, and traveling or exploring new places.
Hannah Meador PT, DPT

Physical TherapistHannah earned her Doctorate of Physical Therapy from Indiana University. Prior to earning her DPT she earned her Bachelor’s of Science in Human Nutrition, Foods, and Exercise from Virginia Tech. While earning her bachelor’s degree, Hannah pole vaulted for the track and field team at the D-I level.
She plans to get certified in dry needing and manual therapy. She enjoys working with young and aspiring athletes, as well as promoting healthy and active lifestyles for older adults.
When not in the clinic, Hannah enjoys spending time outside hiking, kayaking, and playing with her dachshund. She also coaches middle and high school pole vault for the track and field team.

Physical TherapistBob has been a physical therapist, athletic trainer and pedorthist for over 40 years. He has held several appointed and elected offices in the Sports Physical Therapy section of the American Physical Therapy Association. He has served as a clinical consultant for Dr. Scholl’s foot care products and has been involved in their research and development (including performing several clinical studies) of new products.. He has also co-directed professional foot orthotic seminars at over 30 national locations, including all three of the US Olympic Training Centers. He was a custom foot orthotic fabricator for both the Miami Dolphins and the Dallas Cowboys NFL teams. Honors include Indiana state clinical athletic trainer of the year, Indiana state PT award for ‘giving back to the profession’, Confluent Health National Ring of Honor recipient, and the inaugural ‘Friend of the University of Evansville PT programs’ annual honoree.
Nicole Richardson MSOT, CPMT
Nicole Richardson MSOT, CPMT
Nicole Richardson received her Bachelor of Science in Exercise Science with a minor in Massage Therapy in 2017 and graduated with her Master of Occupational Therapy in 2019 at Indiana State University.
Nicole is also a Certified Pediatric Massage Therapist with a passion to serve children in the orthopedic and musculoskeletal setting by improving various physical, developmental, and emotional conditions.
Outside of work, Nicole enjoys spending time with family and friends, being a mom, doing DIY projects, and golfing with her husband.

Clinic ManagerMatt earned his Doctorate in Physical Therapy (DPT) from the University of Indianapolis. Prior to earning his DPT, he received his Bachelor of Science in Exercise Science from the university of Indianapolis. Matt has also completed his orthopedic residency through Evidence In Motion (EIM) and is a Orthopedic Clinical Specialist (OCS).
Matt joined ProRehab in 2012, working in both Indiana and Kentucky. Matt has worked as a therapist in the clinic and home health. He enjoys helping people improve their quality of life and getting them back to their full potential.
In his free time, he enjoys being with his wife, Christie, and three children. He also enjoys cycling, running, and baseball.
Robert Stephen Meyer PT, DPT

Physical TherapistStephen is a two-time graduate of Western Kentucky University. He finished his bachelors of Science in Agriculture and studies in Exercise Science, and then he completed his Doctorate in Physical Therapy in 2023. At this time, he is certified for functional dry needling and is enthusiastically working towards additional certification through continuing education.
In his free time, he enjoys playing video games and board games with friends, listening to audiobooks, cooking new recipes with his wife, and spending time with his cat. In the summer he picks up distance running and watching soccer.

Physical Therapist, Clinic DirectorJay received his Bachelor of Science degree from the University of Kentucky, where he majored in Kinesiology/Exercise Science and then went on to earn his Doctor of Physical Therapy (DPT) degree at Bellarmine. In continuing his education, Jay completed the Orthopaedic Physical Therapy Residency Program through Evidence In Motion and obtained his Orthopaedic Clinical Specialist (OCS) board certification.
Jay is certified in Dry Needling Levels 1 and 2 and has completed the Functional Dry Needling Specialist Certification through Evidence in Motion. Jay is also certified in Blood Flow Restriction Training (BFRT) and has participated in continuing education courses regarding pain neuroscience education and chronic pain management
Jay enjoys treating a wide variety of orthopedic conditions with musculoskeletal dysfunction. He also takes a special interest in performing movement screens, injury prevention screens, and rehabilitation for return to sport within the athletic population.
Shannon Walker PTA
Shannon Walker PTA
Shannon received her Associate of Science degree from Vincennes University and graduated in 2020 as a Physical Therapist Assistant (PTA). Shannon advanced her clinical skills by becoming certified in Selective Functional Movement Assessment (SFMA) Level 1 and the pain neuroscience institute.
Shannon takes a special interest in getting to know her patients and getting them back to what they love. She enjoys staying current on best practice treatment methods with a passion for orthopedic and neuroscience therapy. Previously she volunteered for Rocksteady Boxing which is for individuals diagnosed with Parkinson’s Disease.
Shannon is married to her husband, Terry of 18 years and they have one daughter, Sydney. They enjoy taking family trips to museums, zoos, and taking their dog, Cambridge, to the park. They also enjoy playing board games and watching movies together.

Physical TherapistOriginally from Effingham, IL, Hunter earned her Bachelor’s degree in Exercise Science at the University of Illinois at Springfield where she was also a member of the softball team. Following a career-ending injury, she discovered a passion for injury prevention and rehabilitation treatment. She then went on to graduate from Belmont University with her Doctorate in Physical Therapy. She also holds a designation as a Certified Strength and Conditioning Specialist (CSCS).
Hunter is passionate about collaborating with her patients and helping them reach their goals. She seeks to assist patients in feeling confident, strong, and capable. She now resides in Effingham with her husband, Ian. In her spare time she enjoys reading, trying out new recipes, being outside, and spending time with family and friends.
Sydney Altmeyer OTR/L

Sydney Altmeyer earned her Master of Science in Occupational Therapy from the University of Southern Indiana. Prior to that, Sydney obtained her Bachelor of Science in Exercise Science and Psychology from the University of Southern Indiana.
Sydney specializes in orthopedic conditions of the upper extremity and is most interested in treating said conditions.
In her free time, Sydney enjoys spending time with family and friends, reading, and playing pickleball.

I’m from Leavenworth, Indiana where I grew up playing basketball and volleyball in high school. I received my associate’s degree as a Physical Therapist Assistant (PTA) at Ivy Tech in May 2022. I was a tech for ProRehab New Albany while attending PTA school and had a great experience, so I’m super excited to be back, but now as a PTA.
During my free time, I enjoy exercising, reading, and spending time with friends and family.

Avery Herr earned her Physical Therapist Assistant (PTA) degree from Vincennes University. Prior to that, Avery obtained her Bachelor of Science in Kinesiology from Indiana University – Bloomington.
Avery is most interested in treating orthopedic conditions.
In her free time, Avery enjoys running, reading, and traveling.

Physical Therapist AssistantArye graduated from Vincennes University as a Physical Therapist Assistant (PTA). Previously, she attend the University of Southern Indiana and graduated with her Bachelor of Science in Psychology with a Minor in Sociology. As a PTA and Certified Yoga Teacher (CYT), Arye really enjoys incorporating yoga into physical therapy, along with breathing techniques to help chronic pain patients. She also has her Lifestyle Medicine Certification (LSMC) and is passionate about nutrition and living a healthy lifestyle.
In her free time, Arye loves spending her time with her significant other and their dog, Kai. She enjoys the outdoors, scrapbooking, playing cards or board games with family, and working out.
Aubrey Brazier PTA
Aubrey Brazier PTA
Aubrey graduated from the University of Evansville with a Bachelor’s degree in Exercise Science and an Associate’s degree in Physical Therapy. She also received her certification as a personal trainer. Aubrey has worked in a variety of different rehabilitation settings including orthopedics, skilled nursing, and pediatrics.
Aubrey enjoys working in outpatient therapy because she gets to work with a variety of people with a variety of different diagnoses and loves to continue to grow and learn in her field.
Aubrey now lives in the Northside of Evansville where she loves to work out, play with her dogs, read, and garden.
Kayla Lydon PT, DPT, OCS
Kayla Lydon PT, DPT, OCS
Kayla Lydon earned her Doctor of Physical Therapy (DPT) from Belmont University. Prior to that, Kayla obtained her Bachelor of Science in Kinesiology from Indiana University.
Kayla specializes in post-operative and non-operative orthopedics, and vestibular and is most interested in treating knee pain.
In her free time, Kayla enjoys traveling, hiking, and spending time with family, friends, and her dog!
